Telehealth
HIPAA compliant Telehealth visits are available for follow up visits, unavailable for new patient evaluations, not appropriate for medical emergencies. Telehealth visits are at the discretion of your provider based upon the best quality of your care.
If this is an emergency or you or someone you know are experiencing thoughts of suicide, self harm or harm to others contact 911 immediately or go to your nearest hospital emergency room.
All initial assessments will be in-person in our office.
Telehealth is provided as a service which may not be appropriate for all visits and will be deemed appropriate by your provider.
Minors (under the age of 18) must be seen in-person in our office and accompanied by a parent or an adult guardian.
To utilize Telehealth services, the Telehealth release / information form must be read and signed before the Telehealth visit. This form is available under the "Forms" tab.
In order to participate in our innovative, secure and convenient Telehealth services, it is important that each patient:

agrees this is not a medical emergency. If so contact 911 immediately

choose a location with strong internet connection and capability

choose a private location where you are able to speak freely

login to the Telehealth portal at the time of your scheduled appointment

be fully clothed
-
be stationary / not driving during your appointment

if you are a minor, make sure you are accompanied by a parent
To be able to utilize our Telehealth services, a Telehealth release / information form must be read, agreed with and signed prior to visit.
